  • Aadhaar card or any Photo Identity Card (Original + Xerox)
  • Admit Card of Pharmacy Entrance Test for PET Candidates
  • Recent Pass port size photograph of the candidate (2 copies + soft copy)
  • Domicile Certificate (WB Domiciled Candidates)
  • Caste (SC/ ST/ OBC-A/ OBC-B) Certificate, if allotted through one of the said quotas (Original + Xerox + Softcopy)
  • Physically Challenged Certificate (If allotted by PC Quota) (Original + Xerox + Softcopy)
  • Antiragging Affidavit
